When Padmas pregnancy takes a dangerous turn, the doctors come together to find a solution and turn to Ian to perform a miracle. Meanwhile, Conrad makes a decision regarding his love life and Devon leads his own clinical trials at Chastain.

An attack by cyber criminals sends the Chastain ER into a tailspin and Kit tries to figure out whether or not to pay the ransom. The Raptor deals with a personal connection to the patients brought into the ER during the attack, leaving them all vulnerable. Meanwhile, Conrad works on balancing life as a new dad and Devon and Leela debate taking the next step in their relationship.

After a long wait, Conrad and Nic prepare for their wedding day with the support of their family and friends. Meanwhile, flashbacks to the early days of the COVID-19 crisis shine a light on the heroism of the doctors and the toll the virus took on everyone at Chastain Memorial.

The doctors of Chastain are surrounded by new rules and doctors, as Red Rock Mountain Medical takes over the hospital. Conrad finds himself in a dangerous situation when the construction for a new neurosurgery center causes a gas explosion.

Bell orders the staff to stop accepting new patients and transfer those on life support, but when a child in need is rushed to the ER, Conrad and Devon go against the mandate. Meanwhile, Mina and Austin are tasked with performing heart surgery on a premature baby, despite the lack of power, and Nic works tirelessly to organize the staff and patients who stay during the blackout.

On his first day as an intern, an idealistic Dr. Devon Pravesh faces the harsh realities of medical care, as third year resident Dr. Conrad Hawkins takes him under his wing and teaches his unconventional ways for treating patients. Meanwhile, Conrad goes head-to-head with Chief of Surgery Dr. Solomon Bell, who uses power and intimidation to cover up his mistakes.

The high stakes governor election results in violence at the polls, causing multiple victims to arrive at Chastain. Bell grows frustrated when Marshall Winthrop, Conrads dad and new chairman of the board, requests Conrads input in his discussions about the hospitals expenses. When Cain tries to be a hero at a crash scene, he is struck by a car, causing Chastains doctors to put aside their personal issues to try and save his life. Chastain is flooded with concert goers when a stampede at a music festival grounds leaves many in need of doctors. As Conrad and Nic race to help Josh Robinson, a professional adventure guide with a long list of prior injuries, Devon works with a musician who swears he is sober, even though his toxicology report suggests otherwise. Meanwhile, Bell stirs the pot when the hospitals revered orthopedic surgeon, Dr. Kit Voss, demands a better resident, Julian works to get her boss on Bells good side and Winthrop continues to push Conrad to believe hes a changed man.

4 - Identity Crisis

After a mix-up in the ER causes Conrad to deliver a death notification to the wrong family, he sets out to fix it. Meanwhile, Nic struggles to find a patients records who has been under Lanes care, Devon forms a bond with an elderly patient and Mina is benched after overreaching in the surgical unit.

A college professor comes into the ER and Conrad realizes its a former patient of his from when he was an intern. Meanwhile, Devon ends up admitting a man – who swears by an anti-aging regimen that he thinks is the answer to eternal life – into the hospital after he collapses.

While looking for a distraction on Halloween, Conrad treats a group of witches who send the ER into chaos. Also, Devon treats a patient who thinks he is being haunted by ghosts. Meanwhile, on the way back from his vacation, Bell picks up a hitchhiker who is not what he seems to be.

With Chastain on the verge of shutting down due to the sale by Red Rock, Conrad tries a last-ditch effort to save the hospital and the doctors plan for their next career moves. Meanwhile, on their last day, the whole staff must work together to save Nics life after she is injured by a deranged patient.

On the day of Kit and Bells wedding, Bell and Conrad get pulled away to tend to the ill daughter of a major hospital donor. Meanwhile, Billie takes Gigi and Sammie dress shopping.

Conrad brings a patient into the hospital who causes a high security crisis. An MMA brawl sends a well-known fighter to Chastain and all hands are on deck. Meanwhile, Bell and Raptor vie for the same position at the hospital, and Leela gets a visit from her sister.

As Nic struggles with her recovery, her close friend and former Chastain colleague, Billie, comes back to the hospital to support her. Meanwhile, as the hospital transitions from private to public, the search begins for a new CEO, Bell asks a favor of his step-son and Devon sees an opportunity to fix the system.

On National Nurses Day, the nurses are severely understaffed and overworked. Cain performs a dangerous surgery on a new patient. Conrad discovers that his father has been administered the same drug that he fears played a role in Jessies death.

Halloween at Chastain proves to be just as spooky as ever when Conrad is tasked with diagnosing a young woman admitted to the hospital with extreme night terrors. Meanwhile, Devon faces his first Halloween in the ER, Nic panics over her decision to bring her newly sober sister to Minas epic annual Halloween party and Bell finds an interesting way to make use of his new assistant.

6 - No Matter the Cost

As Conrad battles a group of overly aggressive attending physicians on behalf of a patient with a mysterious chronic ailment, he also must face off against his father who has shown up at the hospital unannounced. When Mina goes out of her way to help a young man from her neighborhood who almost mugged her, Nic offers her assistance and discovers a secret that Mina has been keeping behind closed doors. While away from the hospital, Nic enlists the help of Devon to keep an eye on Lily, which leads him to an incredible find that has him questioning Dr. Lanes motives. Also, Renatas plans to poach a renowned surgeon from another hospital hit a standstill when Bell decides he doesnt agree with him.

7 - The Chimera

Devon sees a traveling British couple who are nervous about the cost of American health care.

7 - Who Will You Be?

A new group of interns, which include Billies son, Trevor, start their first day with Devon at Chastain. Kit and Bell try to convince Conrad to come back to Chastain and The Raptors mom ends up back in the ER. Meanwhile, Leela and Padma deal with an unexpected visit from their parents.

7 - Hero Moments

When Conrad gets a call from his former army commander who is stranded and wounded in the forest, the trip to save him brings lots of old emotions to the surface. Devon and Kit treat a patient who suffers from sickle cell anemia and encourage her to have a hip replacement to help treat her pain. Meanwhile, The Raptor is pulled away in the middle of an intense surgery, Mina is left to put her skills and finish the surgery on her own and Bell works on reconnecting with his stepson after helping him get hired at the hospital.

7 - Woman Down

The doctors work to save one of their own but disagree over whats best for her. Bell is met with an unexpected response when he voices concerns regarding the nursing shortage that Nic brought to his attention.

7 - Trial & Error

When patients from the hospitals prescription drug trial start suffering life threatening side effects, Nic fears that her sister Jessie may be in danger. As a result, she and Conrad work to convince Bell that these trials are not worth the risk. Meanwhile, Mina, Austin and Kit must set aside their competitive nature and work together on a patient whose health is quickly deteriorating and Devon uses Julian as the beta tester for a product hes designed.

7 - The Elopement

Conrad goes head-to-head with Lane when he disagrees with her over a treatment plan for a terminal cancer patient. Tensions rise when Jude inserts himself in the middle by offering to perform the invasive procedure Conrad is fighting against. When a VIP patient crosses the line with Nic, she must devise a strategy on how to stand her ground and combat the situation. Meanwhile, Bell realizes hell need Minas hands more than ever

8 - Old Dogs, New Tricks

Kit hires a surgical coach to watch over Bell and Raptor and help them brush up on their skills. Upon receiving attention from multiple women at the hospital, Conrad contemplates jumping back into the dating scene. Meanwhile, Devon works with an elderly couple.

Devon and Irving are inundated with patients in the ER. When an A-list celebrity enters the hospital after swallowing a wishbone, Logan Kim assigns a new surgeon to the case in the hopes of garnering extra publicity.

8 - Heart in a Box

Conrad goes head-to-head with Bell after he recommends one of Julians risky medical devices to Zoey, whos already lost one son and is back in the hospital with the other. Nic fears that Conrad may be getting overly attached to his patient. Meanwhile, Mina and Austin grow closer while working on a difficult heart transplant, Devon embarks on a very difficult day and Dr. Lane Hunter returns.

8 - Family Affair

Much to Bells dismay, Nic and Conrad decide to admit a homeless Jane Doe, whom they suspect was dumped off by another hospital during Chastains big Charity Ball. Elsewhere in the hospital, Devon and Irving care for a male escort who offers surprisingly good advice, Micah returns for a checkup and falls even harder for Mina, and Devons parents are in town.

Conrad takes Trevor under his wing to care for a patient whose good luck seems to have run out. A case becomes personal for the staff when Conrad and Bell treat a pregnant woman who is carrying Jakes future adopted child. Lane Hunters victims and their families become outraged when she is released from prison. Meanwhile, Conrad uses work to cope with the news about his father, and he teams up with Mina, Kit and the Raptor to treat a patient who is suffering as a result of Lanes dangerous treatments. Then, Devon grows worried when he doesnt hear from Julian, and Nic sets her sights on a primary physician for their new clinic.

Conrad is hit by a bike and badly sprains his ankle, but works through it when he takes the case of his old medical professor, who is hallucinating visions of her old patients. Devon is tasked with admitting Conrad, who refuses any sort of treatment. Nic tries to navigate her emotions while mourning the loss of a patient and Bell operates on a foreign dignitary with unforeseen complications. Meanwhile, Lane and Bells relationship grows closer.

After being terminated, Conrad has difficulty figuring out his next step and distracts himself by accompanying a patient on a zero-gravity adventure. Cain puts the doctors on a commission-based pay system. Bells company gets its big break.

When a medical device fails on a former Chastain employee, an investigation is opened up, which pits Dr. AJ Austin against Quovadis. As Mina and Bell work to get to the bottom of what went wrong, Devon distracts himself with another patient whos experiencing mysterious stomach pains. Meanwhile, Nic forms a bond with her new partner at the clinic and Conrads relationship with the Barnett family and continues to grow.

When Nic finds herself under investigation in Lilys death, Conrad comes to her defense, but ends up making the situation worse. During a routine surgery, Bells patient accidentally catches fire, leaving the hospital to start an immediate investigation into his medical records. However, hospital CEO Claire Thorpe doesnt realize what shes up against when Bell plans a counter attack. Meanwhile, Devon deals with a series of ER patients, including a hypochondriac whom he fears could end up under Lanes care.

With Nics due date steadily approaching, she and Conrad plan a relaxing day off, but their plans are interrupted when a patient with a medical mystery sends them rushing back to Chastain. Devon stays by Roses side as she starts her clinical trial and things take a turn for the worse. Meanwhile, Kit is under pressure to bring Chastain out of debt.

When a nasty flu season turns out to be masking a terribly infectious disease, Chastain must race to quarantine and diagnose infected patients. Mina finds herself testing Quovadis medical devices in an effort to defend Dr. Austin, who, feeling responsible for the death of a patient, is serving penance by staying away from the OR. Conrad is prepared to take the next step with Nic, but finds that she may not be as ready.

12 - Rude Awakenings and the Raptor

Conrad and Devon work tirelessly to save the life of Bradley, a fellow doctor who has fallen through the hospital skylight. As the team contemplates how and why he fell through in the first place, Dr. Bell looks back to when he benched Bradley for falling asleep during surgery. Amidst the madness, Mina is tasked with showing Chastains new star cardiothoracic surgeon, Dr. AJ Austin, around the hospital, but is caught off-guard by his volatile personality. Meanwhile, Nic finds renewed enthusiasm when she uncovers an oncologist who used to work with Lane, but while paying him a visit, she fears that shes being followed.

Conrad meets with a high-profile soccer player suffering from mysterious symptoms. Nic springs into action when she notices Kyle falling into a deep depression and fears he could be suicidal. Devon butts heads with Irving over patient space.

Conrad, Kit, Mina and the Raptor work to save the life of a med student in need of a triple-organ transplant, drawing Bell back to the OR for one of the Chastains riskiest surgeries yet. After the police find Julians empty car in a nearby lake, Devon does some investigating of his own to find out what might have happened. Meanwhile, Mina and the Raptor hound Bell to halt his business with the risky medical device company, Quovadis, and Nic makes a rash decision regarding her sister.

Devon and Conrad race against the clock to save the life of a woman who is deteriorating fast from undetermined symptoms. When the patient turns out to be a hero who helps previous criminals get their lives back together, the team relies on her family of ex-cons to help diagnose and treat her. Tensions rise as Nic continues her search for evidence to incriminate Lane, but must simultaneously tend to her sister, Jessie, who is rushed into the ER. Mina and Devon distract themselves from all Valentines celebrations by trying to figure out how their seemingly healthy neighbor has suddenly passed away.
